Adult Transition Program Criteria

In order for the Adult Transition Program to benefit the students and provide a quality program, the following elements are imperative for a successful outcome.

Student's Profile should include:

  • No major behavior or discipline issues that inhibit employment opportunities.
  • Students must exhibit independence in toileting and feeding.
  • Students are able to transition from the school setting into the community with minimal staff supervision.
  • Staff supervision will fade out and students will be taught to communicate by phone to facilitate student independent skills.
  • Students should not require the support of a SCA (minimum 1 year without the support of a SCA at high school) unless needed for vision or hearing support.
  • Students are able to work independently with minimal prompts.
  • Ability to express their wants and needs in their primary form of communication (verbal or nonverbal) independently.
  • A desire and motivation to be as independent as possible in learning life skills, including job training and employment opportunities, daily living skills and utilizing city transportation.
