Athletic Clearance

Hart Athletics uses an online clearance process. Most of the paperwork required in the past is now completed through AthleticClearance.com. The only exception is the Physical Examination Form.

 

There are two steps for an athlete to clear for participation in summer camps, athletic practice or try-out:

1. Go to AthleticClearance.com.  Set up a user account and password.  Complete the information and electronically sign all required forms.  (Be aware that you will need emergency contact information and proof of insurance to complete the process.  Returning athletes: If you were registered in the system last year, use the same email as before and most of your information will auto-fill!)  Upon completion print the form indicating you have completed the process.  Bring this form to the first day of practice/camp/tryout.    For those who do not have medical insurance there is a link to purchase school insurance embedded in the program.  Insurance is required to participate in High School Athletics. 2. Have a Physical Exam completed by a physician and return the competed Physical Exam Form (Including Medical History) prior to or on the first day of camp/practice.  Note:  All Exams must be completed on or after May 1st, 2024.

Note:  Any Athlete without medical insurance must purchase School Insurance at a minimal cost. There is a link to purchase School Insurance embedded in the AthleticClearance.com process. School insurance may also be purchased at the first day of summer camp. (The cost is usually under $60 and again, is for the entire summer and school year.) You may also visit the Myers-Stevens website and purchase the insurance on-line. www.myers-stevens.com  Please bring verification of purchase to the first day of summer camp. Again, This is only required if the athlete does NOT have medical insurance.
